- The 2007 Medical Home Summit: Small Steps,
Big Changes will take place Thursday, November 15, 2007 at the
Country Springs Hotel in Waukesha, Wisconsin. The summit will highlight
the importance of medical home as quality practice and provide insight
from medical practices currently incorporating medical home concepts
into their daily work. Dr. Carl Cooley from the Crotched Mountain Foundation
in New Hampshire will be the keynote presenter and sessions will include
transitioning from pediatric care to adult care, developmental screening,
